Each patient is prescribed Care Pilot by their physician which educates, guides, and monitors them throughout their journey. By having the physician prescribe the experience, we ensure a high degree of ongoing attachment and engagement to the inforamtion presented to them.

Augment Patient Data

Clarify augments the onboarding data with relevant clinical, claims, demographic, social determinant, and patient-reported data.

While the care team is validating the journey recommendation, the patient is able to use Care Pilot to interact with the patient guide, configure their experience with their support circle, and message directly with the Care Team.
